    Please fill out this form at least 3 weeks prior to your proposed on-campus event. Within 2 weeks of the proposed event, no further changes may be made. The earlier you complete this form, the greater the likelihood of full approval. NOTE: The completion of this form does not guarantee approval for the event. Please await final confirmation from the Student Life Administrative Assistant.
