Event Request Form

This form is used to book venues, order flyers, buy Amazon products for the event, or request use of the Student Life credit card. Submit the Event Request Form at least three weeks before your event.

Reimbursement form

This form is used to process reimbursements. Make sure you obtain itemized receipts from the vendor (credit card receipts are not sufficient).

Tax Exemption Certificate

The seminary has a tax-exempt status. Please bring this certificate with you whenever you are using your personal credit card to buy any food or supplies for your events.
