Ministry Supervisor Form

This form is to be filled out by the student's Ministry Supervisor. It is the student's responsibility to make sure that your Ministry Supervisor submits this form by the appropriate due date. Ministry Supervisors should complete all fields below unless they are serving as part of a lay committee. If the student is being supervised by a committee of lay people, the committee chair should complete the form and may skip questions that do not apply. Please note the requirements below for all persons serving as Ministry Supervisors. Thank you for your partnership with Applied Ministry this semester.
  • Certification

    Ministry Supervisors should hold a Masters of Divinity degree or its equivalent, should possess a minimum of five years experience in vocational ministry, and must read the Applied Ministry Handbook before the start of the semester.
  • Responsibilities

    In addition to the above qualifications, Ministry Supervisors must be willing to (1) meet with the student for a total of at least 8 hours over the course of the semester, (2) discuss the student’s progress, strengths, and weaknesses in a very open and candid manner, and (3) evaluate the student’s progress in their ministry and personal goals through the provided forms.
