Appointments & Payment

Seminarians have access to a limited health maintenance program at the Hagan Clinic, staffed by a charge nurse and physician. Upon presentation of valid identification, current students and their immediate family may use the clinic.

Appointments are scheduled by calling 897-4497 weekdays, and honored according to arrival, yet urgent cases do receive priority. Repeated cancellation results in the loss of future access.

Five dollars is charged for consultation with the physician, and all fees are due at the time of service.

Emergency & Out-Patient Care

For emergencies after office hours, a patient should call 911 or proceed to an intermediate care center or hospital emergency room. Patients are referred for specialized services.

Laboratory tests and X-rays may be requested by the physician, but limited laboratory procedures are offered at the clinic. Conditions needing regular or extensive treatment must be addressed through private arrangements with primary care physicians.

Injections & Physicals

The nurse administers allergy injections 1-4:15 pm (T-Th-F), and 9-11:30 (M) with no appointment needed. Appointments are required for immunizations. Please call 897-4497 to schedule an appointment.

In the absence of a physician’s order, recommendations of the Kentucky Health Department are followed for routine immunizations. School immunization certificates are available, as well as exams for public schools.

Insurance & Records

Students and their dependents should have hospitalization insurance.

A policy may be purchased through GuideStone during matriculation. All clinic charts are filed under the name of the student.

Written requests for the release of records must include the full name of the person under whose name the records are filed, plus the date of graduation/withdrawal.

An Authorization for Disclosure form must be signed in person at the clinic or notarized before records can be released.

Students and their immediate family members are provided a limited health maintenance program through the clinic. Conditions that require regular or extensive medical treatment must be addressed through private arrangements with primary care physician.

It is recommended that every patient have a primary care provider that may be called upon in cases of emergency. The clinic can help with referral to a primary care provider or medical specialist if needed. For more information, contact the Seminary Clinic.

The clinic is closed for all official Seminary holidays, the summer, spring break and fall reading days. Summer hours vary and will be posted accordingly.

Where to go if the clinic is closed

If there is a medical emergency, call 911 or go to the nearest emergency room. If there is a need for medical care contact your primary care provider. If you do not have a provider you may go to one of the nearest urgent care centers (see below).

Appointments

An appointment is required to see the physician. Appointments are made for the day you would like to see the physician.

Patients must present a currently validated ID card. Failure to report without calling is noted, and if repeated, the privilege to make an appointment with the physician is forfeited for the remainder of the semester.

Physicals

Physicals required for entry into public schools are offered. Certificates for required school immunization are provided. Physicals that require an extensive work-up may not be performed. We can provide limited physicals for camp and sports. An appointment is required.
Injections

Injections ordered by a physician are administered weekdays by the nurse. Appointments are scheduled by calling 897-4497.

  • Routine childhood immunizations are given following recommendations of the Kentucky Health Department and Center for Disease Control guidelines. The most current immunization record must be presented on day of visit. Most children qualify for free vaccinations.
  • Adult immunizations are available for adults at cost.
  • Travel vaccines: The clinic offers some travel vaccines for those who will be traveling abroad by following the Center for Disease Control guidelines and recommendations. The clinic does not offer in-depth travel counseling. For more information on travel health visit: www.cdc.gov/travel.
